Transition

Essex County Council's Transition Pathway Service helps young people with a Statement of Special Educational Needs and/or additional needs plan their journey from childhood to adulthood and this is called 'Transition'.  The Transition Pathway Service will work with young people and their families from aged 13 and can offer support up until their 25th birthday.  Most young people on statement will start planning for the future in year 9 (aged 13) and will receive a letter from their school inviting them to a transition review.  The Transition Review will help young people plan for their future and will look at everything from hopes, dreams and aspirations to further education, leisure, health, employment and finance.

Planning for the future and transition review meetings is really important to ensure access to further education and services.

 SEPT (South Essex Partnership University NHS Foundation Trust) is one of the largest and highest performing NHS providers in the country of health and social care services for people with mental health problems and people with learning disabilities.

We provide services across Bedfordshire, Essex and Luton and have a budget of approximately £200 million, employ approximately 3,500 people and serve a population of 1.5 million. Unlike acute hospital services, mental health and learning disability services are mainly provided in community settings with defined geographical localities.

 

west essex nhs   NHS West Essex holds the NHS budget for the 270,000 people who live in Epping Forest, Harlow and Uttlesford. The area we cover stretches from Buckhurst Hill in the south of Essex to Steeple Bumpstead in the north. We are responsible for improving primary care and the health of the local population.  Our annual budget is approximately £330 million.

  Investment in Care staff is essential to maintain the quality of services the sector offers and essexcareconsortium have developed a Training Centre - 'Principles in Practice' (PiP) - that provides the basic and advanced training required by the Care Home Regulations. They are a City & Guilds Approved Centre and provide training for VRQs for a Learning Disability Qualification and NVQs in Health and Social Care and Leadership and Management in Care for Managers.

As well as City & Guilds courses they offer a range of courses covering basic training, physical intervention, new legislation and communication.
